Application Form for a Discovery Call

1. You have to complete each field in the form below and tick the box below for your form to submit.
2. Your information is only seen by me and I will keep it safe. It’s purely for me to know who you are before we talk. Your discovery call is Free and will last for 15 minutes.
3. Once you click the ‘request an appointment button below you will be directed to confirm your
   subscription (this only happens if this is your first time).
4. Go to your inbox and confirm your email address and you’ll be redirected to my calendar to
   book your discovery call with me. If you get redirected to my calendar immediately don’t
   forget to still check your inbox for your confirmation email.
5. Make sure you are prepared so that we can make full use of these 15 minutes.
6. 6. Make sure your mobile number is correct with your country code so that we can have this call.
7. We will be using Zoom for our call so please make sure that you have this app
   downloaded on your phone or computer.
8. If I can’t make the time that you have chosen I will email you to give you another time that is
   more suitable. Please remember to check your email.
